Overview:

Sentara Community Care was created with the intent to invent new and unique models of care that strive to not only address but aid in alleviating some of the socially determined health disparity in our under-resourced communities. Since 2022, this program has expanded rapidly in practice and in location.

We are seeking a seasoned Flex/PRN Nurse Practitioner who shares in our passion and can help us supply high-quality medical care and support to communities that may otherwise not have easy or financial access to medical care.

Position Highlights:

  • Location: 2204 Wilborn Ave, South Boston, VA 24592
  • Scheduling by mutual agreement (Flex/PRN)
  • Current needs include clinic and event coverage averaging about 1 day per week
  • Treating family medicine, pediatrics, and adults with complex issues
  • Shared call schedule, with RN first call
  • Co-manage patients with a diverse care team to include, but not limited to, RN, Counselors, Care Management, and other team members to improve our patients’ health holistically.
  • Review quality and outcome metrics monthly with the healthcare team
  • Collaborate with local civic and faith-based leaders on community education programs
  • Experience working in a FQHC or Community Health Center, or post-masters training in Community Health Care would be of great benefit to the practice
  • 1+ year APP experience required

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $100,900.80 - $312,000.00 annually.

Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
