Always Compassionate Health is looking for caring Home Health Aides (HHAs) and Personal Care Aides (PCAs) to join our growing team. Whether you are looking for a consistent schedule, additional hours, live-in opportunities, or experience supporting clients through NHTD and TBI programs, we want to connect you with cases that fit your availability and experience.

Our caregivers are at the heart of what we do. You will have the opportunity to build meaningful relationships with clients while helping them remain comfortable, safe, and independent in their own homes.

What You'll Do

Following each client's individualized Plan of Care, you may:

• Assist with bathing, grooming, dressing, toileting, and other personal care needs.
• Help clients safely walk, transfer, reposition, and move throughout their homes.
• Assist with meal preparation, feeding, and maintaining proper nutrition.
• Provide light housekeeping and help maintain a clean, comfortable, and safe home environment.
• Provide companionship, encouragement, and emotional support.
• Help clients maintain as much independence as possible.
• Remind clients to take medications as directed in their Plan of Care.
• Assist with approved errands and community activities.
• Support clients participating in NHTD and TBI waiver programs when assigned.
• Use approved medical supplies and equipment when appropriate.
• Assist with treatments or therapy activities when ordered and authorized.
• Observe clients and report changes in condition or behavior to the appropriate clinical team.
• Complete required documentation accurately and on time.
• Communicate with clients, families, nurses, coordinators, and other members of the care team.

Who We're Looking For

We are looking for caregivers who are not only qualified, but who genuinely enjoy helping others.

You should be:

• 18 years of age or older.
• A certified New York State Home Health Aide or Personal Care Aide, as applicable.
• Compassionate, patient, and respectful.
• Dependable and trustworthy.
• A good communicator and listener.
• Comfortable working independently in a client's home.
• Able to follow an individualized Plan of Care and agency policies.
• Professional in appearance and conduct.
• Comfortable working with elderly individuals, people with disabilities, and clients with varying healthcare needs.

Experience with TBI, NHTD, rehabilitation, behavioral health, or community-based care is a plus, but is not required for every case.

Physical Requirements

Depending on the assignment, caregivers must be able to:

• Assist with client transfers, repositioning, and mobility.
• Lift up to 50 pounds when required.
• Bend, stoop, stand, and move throughout a client's home.
• Safely perform the essential responsibilities outlined in the client's Plan of Care.
• Communicate effectively both verbally and in writing.
